Giriş ve amaç: Günümüzde implantların
daha erken yüklemesi ve düşük kalitedeki kemiklerde osseointegrasyon miktarının
arttırılması için ilave tekniklerin kullanılması implant araştırmalarının temel
konularından biridir. Kemik yapımını
stimüle edici çeşitli teknikler hem osseointegrasyon süresini kısaltmak hem de
düşük kalitedeki kemiklerde başarı oranını arttırmak için araştırılmaktadır.
Literatürde osseointegrasyon ve elektrostimülasyon ilişkisini gösteren sınırlı
sayıda çalışma bulunmaktadır. Bu çalışmanın amacı bir elektrostimülasyon türü
olan korona stimülasyonu (KS) nun dental implantların osseointegrasyon kalitesi,
oranı ve sürecine etkisinin deneysel olarak araştırılmasıdır.
Gereç ve yöntem: Eksperimental protokolde dört koça ait 8 tibiyaya toplam 32 implant
uygulanmıştır. Deney hayvanlarının sağ tibialarına KS uygulanmış, sol tarafları
kontrol grubu olarak kullanılmıştır. 15. ve 30. günlerdeki sakrifikasyon periyodunun
ardından nondekalsifiye histomorfometrik yöntemle kemik-implant temas yüzeyi (BIC),
kemik alanı (BAr) ve kemik çecresi (BPm) ölçümleri hesaplanmıştır. İstatistiksel
analizler için SPSS 21 programı kullanılmıştır. İstatistiksel anlamlılık p<0.05 düzeyinde değerlendirilmiştir.
Bulgular: Histomorfometrik
analiz sınucunda BIC, BAr ve BPm değişkenlerinin her biri için deney ve kontrol
grupları karşılaştırıldığında, 15. ve 30. gün gruplarının tamamında deney
grubunda kontrol grubuna oranla istatistiksel olarak anlamlı fark saptanmıştır
(p<0.05). Deney gruplarının 15. ve
30. gün karşılaştırmalarında BIC değeri açısından istatistiksel olarak anlamlı
bir fark saptanmamıştır.
Sonuç: Bu çalışmada KS’nun dental implantların osseointegrasyon sürecinin kalitesi
ve kantitesine iyileşmenin erken dönemlerinde olumlu etkisi histomorfometrik
analizlerle deneysel olarak ortaya konmuştur.

Introduction and aim:
Currently, “the use of additional procedures in order to reduce the loading
time for implants and to enhance osseointegration on poor quality bones” is one
of the most promising research areas in dental implantology. For this purpose,
various techniques have been researched and developed for stimulating bone
production. One of these techniques is electrically stimulating the jaw bone and
surrounding tissues. There is only a limited number of researches on whether
electrostimulation is directly related with osseointegration. The aim of this
experimental study is to investigate the effects of Corona Stimulation (CS) on
osseointegration rate and quality, and whether it decreases the waiting period
of dental implants.
Materials and method:
In our experimental protocol, 32 dental implants were inserted into 4 male
sheep into the tibia bilaterally. Implants on the right tibia of each male
sheep were treated using CS while the other side did not receive any
stimulation and was used as a control group. Animals were sacrificed at 15th
and 30th days after implantation. Bone segments with implants were processed by
noncalcifed method. The determination of the new bone formation and
osseointegration around the dental implants were investigated by means of
undecalcified method, histomorphologically. Mineralized bone-to-implant contact
(BIC), bone area (BAr) and bone perimeter (BPm) percentages were measured by an
experienced blinded investigator to evaluate the bone-implant interface.
Statistical analyzes were achieved by the software SPSS 21 Windows. A P–value less than 0.05 was considered
statistically significant.
Results: The
histomorphometric parameters showed that the BIC, the BAr and the BPm values
have significantly increased in the 15th and 30th days groups in the CS
subjects compared to the control group (p<0.05). No
statistically significant difference in BIC ratio was found between second and
fourth stimulation groups.
Conclusion: The
results of this experimental study indicate that CS may have positive effects
on the early osseointegration period of dental implants.
