Orthodontic-Surgically Assisted Rapid Maxillary Expansion in Skeletal Class II Patient

Introduction: Surgically Assisted Rapid Maxillary Expansion (SARME) is a reliable procedure for the orthodontic treatment of adult patients when it is necessary to expand the maxilla. The aim of this case report is to present the orthodontic and orthognathic surgery treatment approaches of an adult patient with skeletal Class II malocclusion.

 

Case: A 20-year-old female patient applied to our clinic with chief complaint of increased overjet and severe crowding in the lower jaw. In clinical examination Angle class II molar and canine relationships, increased overjet, severe crowding in the lower dental arch, and total circular crossbite were present. In radiological examination, skeletal class II relationship due to mandibular retrognathia, hyperdivergent growth pattern, narrow and hypoplastic maxilla were observed. Buccal corridors were present in smiling. The upper and lower lips were both retrusive.

 

Discussion: Before orthodontic treatment, surgically assisted rapid maxillary expansion (SARME) was performed. After adequate expansion was achieved, four premolar teeth were extracted to eliminate the severe crowding in the lower jaw and to correct the increased overjet. Mini screw assisted distalization was performed in the upper arch. Maximum anchorage was used in the upper dental arch and moderate anchorage was used in the lower dental arch. At the end of the treatment, Angle class I molar and canine relationship, ideal overjet and overbite were achieved.

 

Conclusion: The treatment was successfully completed with the multidisciplinary approach of orthodontics and plastic surgery. A satisfactory, stable, and functional results can be achieved in adult patients with multidisciplinary treatment methods.
