Osseodensifikasyon tekniğinde irrigasyonun sıcaklık artışına etkisi

Özet

Arka plan:

Osseodensifikasyon tekniği, kemik dokunun standart tekniklerde olduğu gibi uzaklaştırılması yerine, osteotomi hattından dışarı doğru sıkıştırılarak kompakt bir yapı haline getirilmesi ve dental implant yuvasındaki kemiğin korunarak bir otojen greft olarak kullanılmasıdır. İmplant osteotomisi sırasında oluşan sürtünme ısısı ve sıcaklık artışı, bu teknikte de implantın primer stabilitesini bozabilecek kemik nekrozu için bir risk faktörüdür.

Amaç:

Osteonekroz alanının genişliği, frezleme sonucu kemikte oluşan sıcaklığa ve termal hasarın süresine bağlı olarak değişir. Bu çalışmanın amacı, osseodensifikasyon protokolü ile implant osteotomisi sırasında periferal kemikte ortaya çıkan sıcaklık değişimlerine irrigasyonun etkisini incelemektir.

Gereç ve Yöntem:

Çalışmamızda insan çene kemiklerini simüle eden farklı dansitelere sahip (D1, D2, D3, D4-Sawbones, Sweden) standardize edilmiş kemik bloklar kullanıldı. Her kemik yoğunluğunda irrigasyonlu ve irrigasyonsuz koşullarda, nihai 3,3 mm frez çapında ve 8 mm frez uzunluğunda osteotomiler yapıldı. osseodensifikasyon tekniği uygulanarak hazırlanan implant yerlerinde oluşan sıcaklık değişimleri in vitro koşullarda incelendi. İmplant yeri hazırlığı esnasında periferal kemikte oluşan sıcaklık değişimleri kızılötesi termal kamera (FLIR E6xt, Estonya) kullanılarak kaydedildi. Deneyler, 2 kg’lık sabit basınç altında yapılırken; eksternal irrigasyon oda sıcaklığında 50 ml/dak sabit hızda serum fizyolojik ile sağlandı. Her kemik yoğunluğunda n (D1) = 20, n (D2) = 20, n (D3) = 20 ve n (D4) = 20 olmak üzere çalışmada toplam N = 80 implant osteotomisi yapıldı

Bulgular:

Çalışmamızda elde edilen verilere göre D1 ve D2 kemik irrigasyonsuz çalışılan grubun sıcaklık ölçüm ortalamaları irrigasyonlu çalışılan grubun ortalamalarından istatiksel olarak anlamlı derecede yüksek bulunurken; D3 ve D4 kemik yoğunluğunda ise irrigasyonsuz çalışılan grubun sıcaklık ölçüm ortalamaları irrigasyonlu çalışılan grubun ortalamalarından istatiksel olarak anlamlı derecede düşük bulundu. En yüksek sıcaklık artış değeri (46,97 °C) D1 yoğunluklu kemikte irrigasyonsuz çalışılan grupta; en düşük sıcaklık artış değeri ise (1,55 °C) ise D4 yoğunluklu kemikte irrigasyonsuz çalışılan grupta kaydedildi.

Sonuç:

Osseodensifikasyon tekniği, kemiği sıkıştırır ve otojen greft olarak şekillendirir. Bu şekilde kemiğin yoğunluğu daha da artmış olur. Özellikle D1 ve D2 gibi yüksek yoğunluklu kemiklerde osseodensifikasyon tekniğinin daha yüksek sıcaklıklar ortaya çıkarması kaçınılmazdır. Çalışmamızın sonuçlarına göre osseodensifikasyon tekniğinde irrigasyon solüsyonu ile soğutma yapılması özellikle D1 ve D2 tip yoğun kemiklerde ısı oluşumunu azaltmada etkilidir. Çalışmamızda D3 ve D4 yoğunluklu kemiklerde yapılan osteotomilerde kemiğin trabeküler yapısı ve eksik olan fizyolojik vücut ısısından dolayı irrigasyonun etkisinin belirgin olarak gözlenmediği düşünülmektedir. Vücut ısısındaki D3 ve D4 yoğunluklu kemiklerde irrigasyon etkisinin daha net bir şekilde ortaya çıkacağı öngörülmektedir.

Background:

Osseodensification is the process of compressing the bone in the dental implant socket out of the osteotomy site into a compact structure and using it as an autogenous graft. Frictional heat and temperature increase during implant osteotomy are risk factors for bone necrosis, which may impair the primary stability of the implant.

Aim:

The extent of the osteonecrosis varies depending on the temperature of the drilled bone and the duration of the thermal damage. This study investigates the impact of irrigation on the temperature that occurs in peripheral bone during implant osteotomy with the osseodensification technique.

Material and Methods:

In this study, we used standardized bone blocks of different densities (D1, D2, D3, D4; Sawbones, Sweden) to simulate human jawbones. The bone blocks were fixed to the drill stand and prevented from moving. The implant contra-angle was fixed to the drill stand so that the implant drill came to the bone blocks at a 90° right angle. To achieve maximum recording sensitivity (maximum accuracy), the infrared thermal camera was placed at a distance of 30 cm, with a height-adjustable holder to ensure precise alignment of the beam with the sample. We performed the osteotomies on the drill stand under the constant pressure of 2 kg with a 3.3 mm drill diameter and 8 mm drill length with-irrigation and no-irrigation conditions at each bone density. External irrigation was provided with saline at a constant rate of 50 ml/min at room temperature. We recorded the temperature changes during implant osteotomy by using an infrared thermal camera (FLIR E6xt, Estonia). A total of 80 implant osteotomies were performed in the study, 20 in D1 density, 20 in D2, 20 in D3, and 20 in D4.

Results:

The key findings we obtained are as follows: First, the mean temperature measurement of the no-irrigation group in D1 and D2 density was statistically significantly higher than the with-irrigation group. Second, the mean temperature measurement of the no-irrigation group in D3 and D4 density was found to be statistically significantly lower than the with-irrigation group. Third, the highest temperature increase (46,97 °C) of the study occurred in the D1 density in the no-irrigation group. Finally, the lowest temperature increase (1,55 °C) was recorded in the no-irrigation group in D4 density.

Conclusion:

Osseodensification technique compresses the bone. In this way, the density of the bone is further increased. Osseodensification technique will inevitably reveal higher temperatures, especially in high-density bones such as D1 and D2. According to the results of our study, cooling with irrigation solution in the osseodensification technique is effective in reducing heat generation, especially in D1 and D2 dense bones. In our study, it is thought that the effect of irrigation was not observed clearly due to the trabecular structure of the bone and the lack of physiological body temperature in osteotomies performed on D3 and D4 density bones. It is predicted that the irrigation effect will appear more clearly in D3 and D4 density bones at body temperature. The maximum temperature reached was always below the critical threshold for osteonecrosis, suggesting that external irrigation is an effective method to prevent excessive heat generation in osteotomies with osseodensification in high-density bones. It is thought that the reason why the effect of irrigation is lower in D3 and D4 density bones is due to the trabecular structure of the bone.
