Özet

Giriş: GLOBOCAN 2018 verilerine göre, kolorektal kanser (CRC) dünyada en ölümcül üçüncü ve en sık teşhis
edilen dördüncü kanserdir. Kolorektal kanser vakası özellikle gelişmekte olan ülkelerde dünya çapında
istikrarlı bir şekilde artmaktadır. Bununla birlikte, erken teşhis ve taramalar ve tedavi seçeneklerindeki son
gelişmeler, artan insidans karşısında bile, gelişmiş ülkelerde CRC mortalitesini azaltmıştır. Genetik test ve
daha iyi aile öyküsü dokümantasyonu, neoplazmaya kalıtsal yatkınlığı olanların önleyici tedbirler almasını
sağlayabilir.
Amaç: Çalışmanın amacı; kolorektal kanserinde kalıtsal kanser sendromlarını tanımlamak, genetik faktörlerin
rolünü göstermek ve tedavi planlamasına yansımalarına dikkat çekmektir.
Yöntem: Çalışma kapsamında İstanbul Üniversitesi, Onkoloji Enstitüsü, Kanser Genetiği Bölümü’ne başvuran
kolorektal kanser tanılı hastalara genetik danışmanlık polikliniği kapsamında genetik yaklaşım stratejilerinin
nasıl olması ile ilgili NCCN kılavuzları dikkate alınmıştır. 17 hastada yeni nesil dizileme yöntemi ile
ABRAXAS1, APC, ATM, BARD1, BRCA1, BRCA2, BRIP1, CDH1, CHEK2, EPCAM, MLH1, MSH2,
MSH6, MRE11A, MUTYH, NBN, PALB2, PMS2, PMS2CL, PIK3CA, PTEN, RAD50, RAD51C, RAD51D,
STK11, TP53, XRCC2 genlerini içeren herediter kanser paneli kullanılarak genetik mutasyonlar
tanımlanmıştır.
Bulgular: Kolorektal kanser tanılı genetik mutasyon taraması yapılmış hastaların %35.3’ünde (6/17) MSH2
(HET, EX:9; c.1436delG pSer479Met fs*3), APC (HET, EX:16, c.7343C>G p.Pro2448Arg rs769559189) ve
CHEK2 (HET, EX:4; c.470T>C, p.Ile157Thr, rs17879961) genlerinde mutasyonlar saptanmıştır. Toplam 6
kişide görülen bu mutasyonlardan biri MSH2 biri CHEK2 genlerinde görülürken APC genindeki mutasyon
aynı aileye mensup 4 kişide saptanmıştır. Bu ailede Ailesel Adenomatoz Polipozis Sendromu olduğu
belirlenmiştir.
Sonuç: Çalışma sonucuna göre kolorektal kanserlerinde kalıtsal kanser genlerinden MSH2, CHEK2 ve APC
genlerinde mutasyon görülmüştür. Böylece kolorektal kanserlerinde genetik mutasyon görülen herediter
kanser genleri ve ilişkili sendromlar tanımlanmış ve ayrıca bu kişilere klinik yaklaşım stratejilerinin nasıl
olması gerektiği belirlenmiştir.

Introduction: According to GLOBOCAN 2018 data, colorectal cancer (CRC) is the third deadliest and fourth
most frequently diagnosed cancer in the world. Colorectal cancer incidence is steadily increasing worldwide,
especially in developing countries. However, recent advances in early detection and screening and treatment
options have reduced CRC mortality in developed countries, even in the face of increasing incidence. Genetic 
testing and better family history documentation can enable those with a hereditary predisposition to neoplasm
to take preventive measures.
Purpose: The purpose of the study; to define the hereditary cancer syndromes in colorectal cancer, to show the
role of genetic factors and to draw attention to their reflections on treatment planning.
Method: Within the scope of the study, NCCN guidelines on how genetic approach strategies should be
considered within the scope of the genetic counseling outpatient clinic for colorectal cancer patients who
applied to Istanbul University, Oncology Institute, Cancer Genetics Department. Genetic mutations were
identified in 17 patients by using a hereditary cancer panel containing that ABRAXAS1, APC, ATM, BARD1,
BRCA1, BRCA2, BRIP1, CDH1, CHEK2, EPCAM, MLH1, MSH2, MSH6, MRE11A, MUTYH, NBN,
PALB2, PMS2, PMS2CL, PIK3CA, PTEN, RAD50, RAD51C, RAD51D, STK11, TP53, XRCC2 genes.
Results: Mutations were found in 35.3% (6/17) of the patients diagnosed with colorectal cancer who were
screened for genetic mutations. These mutations are MSH2 (HET, EX: 9; c.1436delG pSer479Met fs * 3),
APC (HET, EX: 16, c.7343C> G p.Pro2448Arg rs769559189) and CHEK2 (HET, EX: 4; c.470T> C,
p.Ile157Thr, rs17879961). While one of these mutations seen in 6 people was seen in MSH2 and the other was
in CHEK2 genes, the mutation in the APC gene was detected in 4 members of the same family. Familial
Adenomatous Polyposis Syndrome has been determined in this family.
Conclusion: According to the results of the study, mutations were observed in the hereditary cancer genes
MSH2, CHEK2 and APC in colorectal cancers. Thus, hereditary cancer genes and associated syndromes with
genetic mutations in colorectal cancers were identified, and the strategies for clinical approach to these
individuals were determined.
