Aim: Carotd endarterectomy s proved surgcal nterventon for reducng the rsk of stroke n patents wth carotd artery stenoss.
Carotd endarterectomy decreases recurrent stroke rates n symptomatc patents. As the growng proporton of octogenarans n
the ageng populaton may lead to a dramatc ncrease n cerebrovascular dsease, preventng and treatng stroke wll be a serous
challenge n the octogenaran. We revewed the outcome of carotd endarterectomy n the octogenarans wth or wthout contralateral
carotd stenoss or occluson and compared the results wth a smlar cohort of younger age.
Material and Methods: From 2005 to 2025, 440 CEAs were performed by a sngle surgcal team on 360 patents were revewed for
early outcome wth regards to hosptal mortalty and stroke.
Results: Among 360 patents, 78 patents were 80-years old or older (62 males; mean 82.59.3 years-old) underwent blateral
CEAs. Actve smokng was sgncantly hgher n the male octogenarans (6.2 vs. 21.8, P 0.05). Preoperatve hstory of non-
debltatng stroke was the most common scenaro n all patents. Preoperatve hstory of transent schemc events was the most
commonscenaro n both octogenarans and the younger patents although preoperatve transent neurologc de cts were more
promnent n the non-octogenarans (50 vs. 71, P 0.05). 12 patents were presented wth pslateral retnal artery occluson.
Contralateral carotd artery occluson was seen n a total of 40 patents. Only one patent n the octogenaran group experenced a
lateralzng stroke due to pslateral CEA wth contralateral occluson group n contrast to 3 patents (lateralzng stroke n 3 patents,
lacunar state n one patent) n the contralateral stenoss group.
Conclusion: Carotd endarterectomy, despte the general percepton, s a vable opton for patents wth CCO or blateral dsease even
n the octogenaran group.
