Intraoral vs. Extraoral Approaches for the Surgical Treatment of Mandibular Peripheral Osteomas: A Study of 4 Cases

Özet

Abstract: Background

Osteomas are benign osteogenic neoplasms characterized by the proliferation of compact, cancellous, or both types of bone. Peripheral osteomas, although rare, are most commonly found in the maxillofacial region, including the mandible. They typically present as slow-growing expansion, leading to facial asymmetry and, in some cases, functional issues.

Objective

This case series aims to compare the outcomes of surgical treatment for 4 cases of mandibular peripheral osteomas using intraoral and extraoral approaches.

Methods

This case series consists of 4 patients with mandibular peripheral osteomas, and the treatments rendered are supported with a review of the associated literature. The surgical approach (intraoral vs. extraoral) was determined based on the localization and the size of the lesion. Preoperative imaging, including radiographs and CT scans, was used to assess the size and the extent of the lesions.

Results

Intraoral approaches were used in two cases, while extraoral approaches were used in the other two cases. The intraoral approach provided excellent access and visibility for lesions located near the mandibular body and ramus. However, for lesions located in the angle or inferior border of the mandible, the extraoral approach provided better exposure. All patients experienced satisfactory outcomes with no complications during the follow-up period.

Conclusion

The choice between intraoral and extraoral approaches for the surgical treatment of mandibular peripheral osteomas should be based on the location and size of the lesion. Both approaches can provide excellent outcomes with low complication rates when performed by experienced surgeons.
