BACKGROUND AND AIM:The 13-months-old patient with microcephalic osteodysplastic primordial dwarfism type II presented with bilateral congenital cataract and microcornea.She had bilateral canthotomy,lens aspiration and anterior vitrectomy.Because of tight lids,narrow interpalpebral aperture and microcornea she couldn’t use contact lenses and due to microcephaly she couldn’t wear pediatric eyeglasses for optical rehabilitation in the postoperative period (Figure 1).So custom made frames for eyeglasses were produced.
METHODS:IPad Air is connected to Structure Sensor(Occipital Inc).By using the application called Scanner, three-dimensional(3D) model of the face of the patient was captured.The data was transfered to the computer and 3D model of the face was reconstructed(Figure 2).On the reconstructed 3D model, the glasses was designed by using Rhinoceros 3D computer graphics and computer-aided design application software.First the frames was producted by Ultimaker 2+ 3D printer which uses Fused Deposition Modelling technology with 2.85mm Polylactic Acid 3D printing filament.It was tested to fit the face of the patient,and decided to increase the temple length.Also the strength of the product was not sufficient to hold the 22 diopter lenses.So the frames were rebuilt by Formlabs Form2 3D printer which uses stereolithography technology with photopolymer resin.The lenses were cut and inserted into the frame by an optician.Then the glasses were tried on the face of the patient(Figure 2 and 3).In the trial period the temple of the frames fit tightly,so a new frame was designed and hinges were added to temples of the frame(Figure 4).The new frame was producted by 3D printer which uses Selective laser sintering technology with polyamide.
RESULTS:The patient had an ambulatory vision at the end of the 1-year follow-up period.
CONCLUSIONS:Standard pediatric frames are not suitable for all pediatric patients.Custom made frames can be used for the patients with dysmorphic feature for optical rehabilitation.
