Aim: To assess potentially inappropriate medication use (IMU), including both potentially inappropriate prescribing (PIP) and prescribing omissions (PPOs) according to the Turkish Inappropriate Medication Use in oldEr adults (TIME) criteria in older nursing home residents and to investigate its association with common geriatric syndromes. Findings: Both PIPs and PPOs were frequent in older adults, being associated with polypharmacy and frailty, respectively. Common issues included unnecessary antihypertensives, use of neuroleptics as hypnotics, proton pump inhibitor use, and missed treatments for dementia and malnutrition. Message: Polypharmacy and IMU were highly prevalent among older nursing home residents. Our findings emphasize that geriatric syndromes—particularly frailty—were strongly associated with PPOs, as identified by the TIME-to-START criteria.
