Amaç: Hipertansiyon, kardiyovasküler hastalıklar için önemli bir
değiştirilebilir risk faktörüdür. Antihipertansif ilaçlar ve yaşam tarzı
değişiklikleri ile kan basıncının düşürülmesi hedef organ hasarını
azaltabilmekte ve hastalık sonuçlarını engellemektedir. Homosistein kan
plazmasında düşük mikromolar konsantrasyonlarda bulunan, protein kodlamayan,
diyetsel olmayan yolla oluşan sülfürlü bir aminoasittir. Homosistein
metabolizmasında ve rejenerasyonunda görevli önemli bir enzim olan MTHFR
(5,10-metilentetrahidrofolatredüktaz) C677T ve A1298C gen varyasyonlarının
etkisi kanser, miyokard infarktüs, diyabet, inme, renal hastalıklar, konjenital
defektler, koroner kalp hastalığı, depresyon ve şizofreni gibi birçok
hastalıkla ilişkilendirilmiştir. Çalışmamızda, hipertansiyon hastalarında MTHFR
C677T ve A1298Cpolimorfizmleri ve homosistein serum düzeyi belirlenerek bu
polimorfizmlerin homosistein serum seviyesi ve hipertansiyon üzerine etkisinin
incelenmesi amaçlanmıştır.
Metod:
Çalışmaya hipertansiyon tanılı 80 birey (41 kadın ve 39 erkek) ve sağlıklı 67
birey (36 kadın ve 31 erkek) dahil edilmiştir. Bireylerden alınan periferal kan
örneklerinden genomik DNA izolasyonu yapılarak DNA konsantrasyonu ve saflığına
nanometre cihazında bakıldı. Öncelikle MTHFR geni PZR (polimeraz zincir
reaksiyonu) yöntemi ile çoğaltıldı. Sonra MTHFR geninin C677T ve A1298C
polimorfizmlerine bakılması için RFLP (restriksiyon fragment uzunluk
polimorfizmi) yöntemi ile kesim protokolü gerçekleştirildi ve polimorfizmler
agaroz jel elektroforezinde saptandı. Bireylerin serum örneklerinde ELISA
(enzime bağlı immunosorbent assay) yöntemi ile homosistein serum seviyeleri
ölçüldü. Sonuçlar SPSS 22 programı ile analiz edildi ve p˂0,05 istatiksel
olarak anlamlı kabul edildi.
Bulgular:
MTHFR C677T genotip dağılımı incelendiğinde hasta ve kontrol grupları arasında
istatistiksel bir anlamlılık gözlendi (p<0,001). Kontrol grubunda en fazla
CC (%61,80) genotipi gözlenirken, hasta grubunda ise en fazla TT (%79,60)
genotipi gözlenmiştir. MTHFR A1298C genotip dağılımı incelendiğinde ise, hasta
ve kontrol grupları arasında istatistiksel bir anlamlılık gözlenmemiştir
(p=0,058). Hasta ve kontrol grupları genotip dağılımları incelendiğinde
birbirine yakın oranlar olduğu görülmüştür. Allel frekans dağılımları incelendiğinde
hem MTHFR C677T hem de A1298C allel frekansı dağılımlarında istatistiksel
olarak anlamlılık gözlenmiştir (sırasıyla p<0,001, p=0,005). Kontrol
grubundaki serum homosistein seviyeleri 19,41±1,76 μmol/L, hasta grubunda ise
24,63±2,18 μmol/L olarak bulunmuş olup istatistiksel olarak anlamlı bir
farklılık görülmemiştir (p=0,065). Hasta ve kontrol gruplarında MTHFR C677T ve
A1298Cpolimorfizmleri genotip frekans dağılımlarına göre serum homosistein
seviyelerinin anlamlılığına bakıldığında C677Tgenotip frekans dağılımları ve
homosistein seviyeleri arasında istatistiksel olarak anlamlılık gözlenmiştir
(p=0,027).Fakat, A1298C genotip frekansları ve homosistein seviyeleri arasında
istatistiksel olarak anlamlılık gözlemlenmemiştir(p=0,996).
Sonuç:
Çalışmamızda MTHFR 677TT genotipinin, hipertansiyon patogenezinde önemli rol
oynayabileceği gösterilmiştir. Hipertansiyonun klinik değerlendirilmesinde yeni
bir belirteç olabilir. MTHFR 677TT genotipinin homosistein seviyesinde artışa
neden olduğu da gösterilmiş olup, homosistein seviyesini düşürücü tedavilerin
belirlenerek hipertansiyon insidansında azalma olacağı kanısındayız.
Aim: Hypertension (HT) is one of the most important modifiable
risk factors for cardiovascular disease. Lowering of blood pressure through
antihypertensive drugs and lifestyle changes can reduce target organ damage and
prevent disease outcomes. Homocysteine is a non-protein coding, non-dietary and
sulphurous amino acid that is found in low micromolar concentrations in the
serum. The effect of MTHFR (5,10-methylenetetrahydrofolate
reductase) C677T and A1298C gene variations, an important enzyme involved
in homocysteine metabolism and regeneration, has been associated with many
diseases such as cancer, myocardial infarction, diabetes, stroke, renal
diseases, congenital defects, coronary heart disease, depression and
schizophrenia. In the our study, it is aimed to investigate the possible
effects of this variations on homocysteine serum level and hypertension by
determining MTHFR C677T and A1298C polymorphisms and homocysteine serum level.
Method: 80 hypertension patients (41 women and 39 men) and 67
healthy control individuals (36 women and 31 men) were included in the study.
Genomic DNA isolation was performed from peripheral blood samples taken from
individuals, and DNA concentration and purity were measured in a nanometer
device. Firstly, MTHFR gene was amplified by PCR (Polymerase Chain Reaction)
method. In order to determine the polymorphisms of MTHFR C677T and A1298C, RFLP
(Restriction Fragment Length Polymorphism) method was performed and the
polymorphisms were determined in agarose gel electrophoresis. Serum
homocysteine levels were measured by ELISA (enzyme-linked immunosorbent assay)
technique. The results were analyzed by SPSS 22 program and p˂0.05 was
considered statistically significant.
Results: In the MTHFR C677T genotype distribution, a statistical
significance was observed between the patient and control groups (p <0.001).
While CC (61.80%) genotype was observed the most in the control group, TT
(79.60%) genotype was observed the most in the patient group. In the MTHFR
A1298C genotype distribution, no statistical significance was observed between
the patient and control groups (p=0,058). The genotype distribution rates of
the patient and control groups were close to each other. When the allele
frequency distributions were examined, statistical significance was observed in
the both MTHFR C677T and A1298C allele frequency distributions (respectively
p<0,001, p=0,005). Serum homocysteine levels were measured as 19.41 ± 1.76
μmol/L in the control group and 24.63 ± 2.18 μmol/L in the patient group but no
statistical significances were observed (p=0,065). When the significance of
serum homocysteine levels according to the genotype frequency distributions
of MTHFR C677T and A1298C polymorphisms in the patient and control groups, a
statistical significance was observed between the C677T genotype frequency
distributions and homocysteine levels (p=0,027). However, no statistical
significance was observed between the A1298C genotype frequency distributions
and homocysteine levels (p=0,996).
Conclusion: In our study, it has been shown that the
MTHFR 677TT genotype may play an important role in the pathogenesis of
hypertension. It may be a new marker in the clinical evaluation of hypertension.
It has also been shown that the MTHFR 677TT genotype causes an increase in
homocysteine level, and we believe that the treatment that lowers the
homocysteine level will be determined and the incidence of hypertension will
decrease.
