Abstract
Aim: Endovascular treatment of symptomatc perpheral artery dsease has ganed wdespread acceptance. The ecacy and safety
of drug coated balloon angoplasty n the settng of crtcal lmb schema n comparson to conventonal surgery was prevously
demonstrated wth favorable 1-year outcomes. Evdence however remans lmted on the durablty of the treatment n the longer
term. We have compared our 4-year long results wth DCB angoplasty to conventonal bypass surgery n patents wth crtcal lmb
schema.
Material and Methods: A total of 187 patents wth CLI treated over a 6-year perod between 2006 and 2012 by a sngle operatve
team consttuted the study populaton. Between 2006 and 2009, all patents underwent conventonal surgery. Between 2009 and 2012,
the nvestgators adopted endovascular approach wth the use of IN.PACT Admral (Medtronc Inc., Santa Rosa, CA, USA). Data
collecton was acheved prospectvely.
Results: A total 210 procedures (100 surgery, 110 endovascular) were preformed over a 6 year-perod. A 72 of all bypasses were
performed usng saphenous ven grafts wth above-knee bypass as the technque of choce n 80 of the cases. 6-mm DCB was used
n 41 of the patents. Procedural success rates (98 vs 99, NS) as well as clncal success rates (99 vs 99, NS) and operatve
mortalty (3.7 vs 2, NS) was smlar n both groups. Prmary Patency for DCB vs. bypass groups 91.8 vs. 88.9, respectvely
(P0.31) at 12 months; 82.7 vs. 82.8 at 24 months, (P0.28) and remaned smlar at 4 years (66.4 vs.68.8, P0.08), respectvely.
Freedom from clncally-drven TLR at 12 months was smlar n both groups (87.6 vs. 85, P0.33). The rates of clncally drven
target leson revascularzaton were 29.2 and 26.1 (P0.07) for the DCB and surgery groups, respectvely.
Conclusion: Drug coated balloon angoplasty yelds comparable results to surgery n the settng of crtcal schema even at 4 years.
The ecacy and the safety of DCB's n more complex lesons s to be nvestgated wth randomzed trals.
