Amaç: Bu olgu sunumunun amacı dişlerdeki madde kaybının restoratif tekniklerle, dişeti
çekilmelerinin ise bağ dokusu greftiyle birlikte kuronale konumlandırılan flep tekniği kullanılarak
tedavisidir. Yöntem ve Bulgular: Mandibular anterior 33-43 numaralı dişler arasında restoratif ve
periodontal cerrahi tedavi ihtiyacı olan 31 yaşındaki sistemik olarak sağlıklı erkek hasta, Fakültemizin
Peridontoloji Anabilim Dalı’na dişlerinin görüntüsünden ve dişeti çekilmelerinden rahatsız olması
nedeniyle için başvurmuştur. Yapılan klinik ve radyolojik değerlendirmeler sonucunda hastanın kök
yüzeyindeki defektlerinin kompozit restorasyonları tamamlanmış, dişeti çekilmesinin eşlik ettiği
bölgelerde restorasyonun periodontal cerrahiyle eş zamanlı yapılması planlanmıştır. 33-43 numaralı
(FDI) dişler arasına Langer ve Langer (1985) tekniğiyle dişeti altındaki defektlere ulaşmak amacıyla
flep açıldı. Kök yüzeylerinin restorasyonundan sonra 17- 13 numaralı (FDI) dişler arasından palatinal
damak bölgesinden 30 mm uzunluğunda 3 mm genişliğinde serbest dişeti grefti alındı ve deepitelize
edildi. Bağ dokusu 5/0 rezorbe olan multifilament dikişle, flep ise 6/0 rezorbe olmayan monofilament
dikişlerle sabitlendi. 2 hafta sonra dikişler alındı. Ameliyat sonrası post-operatif kontrol randevusunda
doku iyileşme yanıtı olumlu olarak gözlemlenmiştir. İyileşme tamamlandıktan sonra bazı bölgelerde
tam kapanmayan kök yüzeyleri olduğu izlenmiştir. Sonuç: Dişeti çekilmeleri birçok etiyolojik faktöre
bağlı gelişebilir. Bu çekilmelerinin ardından açığa çıkan kök yüzeyinin kapatılması, çekilmeye neden
olan etyolojik faktöre göre farklı yaklaşımlarla tedavi edilebilir.
Objective: The purpose of this case presentation is to describe the treatment of tooth substance loss
with restorative techniques and gingival recession with connective tissue grafting using the coronally
positioned flap technique in a multidisciplinary approach. Method and Results: A systemically healthy
male patient, aged 31, with restorative and periodontal surgical treatment needs between mandibular
anterior teeth 33-43, sought treatment at the Periodontology Department of our Faculty, due to
dissatisfaction with the appearance of his teeth and gingival recessions. After non-surgical periodontal
treatment, the patient was referred to the Department of Restorative Dentistry and composite
restorations were completed on the root surfaces that did not extend to the gingiva. At the same time,
restorations were planned for the subgingival defects. A flap was raised using the Langer and Langer
technique (1985) to access the subgingival defects between teeth 33-43 (FDI). After restoration of the
root surfaces, a 30 mm long and 3 mm wide free gingival graft was harvested from the palatal region
between teeth 17 and 13 (FDI) and deepithelialized. The connective tissue was fixed with 5/0
absorbable multifilament sutures and the flap was secured with 6/0 non-absorbable monofilament
sutures. Two weeks later, the sutures were removed and the post-operative follow-up showed a positive
tissue healing response. Upon completion of the healing process, some sites presented incompletely
closed root surfaces. Conclusion: Gingival recessions can develop due to various aetiological factors.
After these recessions, the exposed root surface can be treated using different approaches depending
on the etiological factor causing the recession.
