Abstract

A 28-year-old male presented with chronic low back pain persisting for two years following a civilian gunshot injury, with retained shrapnel fragments in the L2 vertebra. Despite multiple outpatient evaluations and extensive pharmacological treatments, including nonsteroidal anti-inflammatory drugs, gabapentinoids, antidepressants, and opioids, his pain remained inadequately controlled. Due to the presence of retained spinal foreign bodies, epidural interventions were avoided. As an alternative approach, an ultrasound-guided erector spinae plane block (ESPB) was performed at the T12 level. The patient experienced immediate and significant pain relief,
