Özet
Amaç:
Medyan rafe kistleri, erkek genital organlarının nadir görülen benign lezyonlarıdır.
Embriyolojik geliĢimsel anomaliler olarak kabul edilirler ve penis ucundan anüs kenarlarına
kadar olan bölgelerde ortaya çıkabilirler. Bu yazıda skrotal lezyon olarak baĢvuran bir
medyan rafe kisti olgusunu tartıĢacağız. Bu anatomik bölge, bu tür epidermal kistler için de
nadir lokasyonlardır.
Olgu Sunumu:
Bizim olgumuz skrotal bölgede 2 aydır olan ĢiĢlik öyküsü ile baĢvuran 3 yaĢında bir erkek
çocuk. Fizik muayenesinde skrotal orta hatta 2 cm uzunluğunda perineal ĢiĢlik mevcuttu.
Lezyon ile iliĢkili herhangi bir enfeksiyon veya akıntı belirtisi yoktu. Ayırıcı tanıda
anokütanöz fistül, skrotal fistül veya izole lezyon bulunmaktaydı. Skrotal US; inferior skrotal
subkütan 18x5.5 mm düzgün sınırlı izoekoik homojen solid lezyon olarak raporlandı. Üriner
US ve voiding sistoüretrogramda anormal bulgu saptanmadı. Tanıyı ve tedavi planını
netleĢtirmek için genel anestezi altında muayene ve sistoskopi planlandı. Perioperatif
muayenede üretranın intakt olduğu, transüretral sistoskopide ise üriner sisteme uzanma
olmadığı saptandı. Anokütanöz fistül tanısı da dıĢlandı ve lezyonun izole bir kist olduğuna
karar verildi. Kist eksize edildi ve çıkarılan materyal histolojik inceleme için patolojiye
gönderildi.
Sonuç:
Medyan rafe kistleri genellikle asemptomatik lezyonlardır ve nadiren enfekte veya ülsere
olurlar. Genitoüriner ve gastrointestinal kanallar hasta değerlendirmesi sırasında mutlaka
kontrol edilmelidir. Cerrahi eksizyon ilk tedavi seçeneği olmakla birlikte, özellikle
asemptomatik hastalar için müdahalesiz izlem de bir alternatif olabilir.

Introduction:
Median raphe cysts are rare lesions of male genitalia which are benign in nature. They are
considered as embryologic developmental anomalies and can arise along the tip of penis to
the margins of anus. We will discuss a case of median raphe cyst, presented as scrotal lesion;
this anatomical location is also rare for these type of epidermal cysts.
Case Report:
Our case is a 3-year-old boy, presenting with a 2-month history of swelling in the scrotal
region. Physical examination revealed 2-cm-length perineal swelling in the scrotal midline.
No signs of infection or discharge were present related to the lesion. Differential diagnosis
involved anocutaneous fistula, scrotal fistula or an isolated lesion. Scrotal US performed and
was reported as inferior scrotal subcutaneous 18x5.5 mm smooth-contoured isoechoic
homogenous solid lesion. There was no abnormal finding in urinary US and voiding
cystourethrogram. Examination under general anesthesia and cystoscopia have been planned
to clarify the diagnosis, as well as for treatment plan. Perieoperative examination revealed
urethra was intact, transurethral cystoscopia performed and there was no extension to urinary
system. Anocutaneous fistula was also excluded and lesion was decided to be an isolated cyst.
Cyst was excised and excisional material sent to pathology for histological examination.
Conclusion:
Median raphe cysts are usually asymptomatic lesions and rarely become infected or ulcerated.
Genitourinary and gastrointestinal tracts should be checked during patient evaluation. Despite
surgical excision is the first choice of treatment, observation can also be alternative especially
for asypmtomatic patients.
