The value of transluminal extraction atherectomy in the treatment of coronary artery disease: A case report

Özet

Despite technical advances in balloon catheter and guidewire systems and greater experience, the success rate of percutaneous transluminal coronary angioplasty (PTCA) continues to be limited by abrupt closure 3-5 % of cases, and chronic restenosis which occurs in 25-40 % of patients. The new technologies (coronary atherectomy, intracoronary stents, and laser/laser thermal techniques) have recently entered clinical testing with the goal of providing a practical and predictable solution to these difficulties. A 49-year-old man was admitted to our clinic due to acute inferior and right ventricular myocardial infarction. Reperfusion was achieved in infarct-related right coronary artery by IV streptokinase. The coronary arteriography also revealed a 80 percent luminal narrowing in the left anterior descending artery. The combined 'transluminal extraction atherectomy' and PTCA were successfully performed with reduction of the target stenosis to a residual 20 percent narrowing. The method of transluminal extraction atherectomy in coronary arteries was used only in a few patients, and we could not find any publication about it in Turkey, so we decided to report of this case.
