ÖZET Ortodontik diş hareketi, dişi çevreleyen sert ve yumuşak dokuların şekil ve içeriğindeki dinamik değişikliklerin sonucu olarak gerçekleşir. Ortodontik tedavi ile düzeltilen dişlerin tedavi öncesi pozisyonlarına geri dönmesinin (nüks) önlenmesi, ortodontik tedavinin başarısında önemli rol
oynamaktadır. Aktif tedavi süresinin sonunda çeşitli pekiştirme apareyleri kullanılarak ortodontik tedavi sonuçlarının kalıcı olması hedeflenir. Ancak rotasyonlu dişlerde nüks görülme olasılığı, diğer diş
malpozisyonlarına oranla daha fazla olmaktadır. Bunun nedeninin dişlere yaptırılan rotasyonel hareket sonrasında suprakrestal periodontal liflerin ve dişeti liflerinin gerilmesi, liflerin yeni diş konumuna
yetersiz adaptasyonu olduğu düşünülmektedir. Dişe yaptırılan rotasyon miktarı arttıkça liflerdeki gerginlik de artar. Nüksü önlemek için rotasyonlu dişlerde overcorrection (aşırı düzeltme) yapılması, proksimal temas yüzeylerini genişletmek için interproksimal stripping işlemi ve uzun süreli pekiştirme
apareyi kullanımı önerilmektedir. Rotasyonel nüksün önlenmesinde en etkili yöntem fiberotomi işlemidir. Liflerdeki gerilmeyi ortadan kaldırmak ve dolayısıyla nüksü azaltmak için suprakrestal fiberotomi işlemini içeren çeşitli prosedürler önerilmiştir. Cerrahi fiberotomi, nüksün önlenmesinde oldukça
etkilidir. Dişleri tedavi başındaki pozisyonlarına çeken lifler bistüri ile kesilerek liflerin daha az gerilmiş bir konfigürasyonda yeniden oluşmasına olanak sağlanır. Son yıllarda cerrahi yönteme alternatif
olarak lazer destekli fiberotomi teknikleri ve düşük doz lazer tedavisi uygulamaları kullanılmaktadır.
Bu tekniklerin ortak özelliği, suprakrestal ve dişeti liflerinin kesilerek veya bu liflerin remodelingini
hızlandırarak dişin yeni konumuna adaptasyonunu sağlamaktır.

ABS TRACT The process of orthodontic tooth movement is a resultant dynamic change in the shapes
and composition of the investing bone and soft tissues. Preventing the teeth from returning to their
pre-treatment positions (relapse) plays an important role in the success of orthodontic treatment. At the
end of the active treatment period, orthodontic treatment results are stabilised by using various removable or fixed retainers. However, relapse tendency of rotations are higher than other dental malpositions. The reason for higher relapse is thought to be stemmed from stretching of supracrestal
periodontal fibers and gingival fibers after derotational movement of the tooth and the inadequate adaptation of fibers to final tooth position. As the magnitude of rotation increases, the fibers elongate more.
To prevent relapse; overcorrection of rotation, interproximal stripping for larger proximal contact surfaces and long-term use of retainers are recommended. The most effective method of preventing rotational relapse is fiberotomy. Various procedures including supracrestal fiberotomy have been proposed
to eliminate stretch of fibers and thus reduce relapse after derotating teeth. Surgical fiberotomy is effective to prevent relapse. The fibers that stretch teeth back to the pretreatment position, are cut with
a scalpel, allowing the fibers to regenerate in a stable configuration. Recently, laser-aided fiberotomy
techniques and low-level laser therapy applications have been used as an alternative to the surgical
method. All these techniques aim for better tooth adaptation to its new position, either by cutting the
supracrestal and gingival fibers or by increasing remodeling rate of these fibers.
