Background: Gingival recession (GR) is defined as the apical displacement of the gingival margin relative to the cemento-enamel junction, resulting in root surface exposure. GR can lead to several clinical outcomes that warrant treatment, including root dentine hypersensitivity, aesthetic concerns, plaque retention and inflammation, tooth abrasion, and an increased risk of root caries. In this case report, 21-year-old systemically healthy female patient undergoing orthodontic treatment presented with gingival recession during routine dental check-up, highlighting the need for intervention to restore periodontal health and prevent further complications. A free gingival graft was selected as the preferred treatment to achieve effective root coverage and address the patient's clinical needs.
Description of the procedure: A free gingival graft procedure was performed to address the gingival recession in teeth #31, #41, and #42. Following local anaesthesia the recipient site was prepared, and the graft was harvested from the palatal mucosa. Gingival recession measurements were 4 mm for #31, 3 mm for #41, and 2 mm for #42, with keratinised tissue width absent in #31 and limited to 1 mm in #41 and #42.
Outcomes: Complete root coverage was achieved in the grafted area, with gains in keratinised mucosa measuring 4, 5, and 5 mm for teeth #31, #41, and #42, respectively. The patient is attending regular follow-up appointments and reports no complaints.
Conclusions: This case report demonstrates that free gingival graft, with an appropriate indication, is an effective treatment option for both complete coverage of exposed root surface and support of keratinised gingiva.
