Compromised orthodontic treatment in severe deepbite patient:A case report

INTRODUCTION:Correction of the skeletal deepbite has always been a challenge in orthodontics.This case report describes the compromised orthodontic treatment of a patient with Class II severe deepbite.

CASE :A 16-year-old-patient presented with a complaint of difficulty in biting and impingement of the palatal gingiva by the mandibular incisors.Extraoral clinical examination showed a symmetrical face and convex profile,lip incompetence at rest,prominent nose and protruded upper/lower lip.Intraoral clinical examination revealed a severe class II malocclusion with a complete traumatic deepbite and a severe overjet.Radiographic examinations showed skeletal class II with prognathic maxilla and retrognatic mandible,vertical growth pattern and upper/lower incisors retroclination.



DISCUSSION:Surgical-orthodontic approach was previously recommended as the ideal treatment,however,the patient refused it and compromised treatment was planned.The treatment objectives were to correct the deep bite,improve the skeletal pattern,obtain class-I molar and canine on both sides with fixed functional appliance,flatten the curve of spee and create functional occlusion.The deepbite was corrected by extrusion of the mandibular molars using a fixed anterior biteplane appliance with up-down elastics.During the treatment process,the use of the fixed functional appliance was cancelled as the class I canine relationship was achieved by opening the bite and simultaneous movement of the lower arch in the mesial direction and upper incisors retraction and intrusion were achieved by applying posterior miniscrews.



CONCLUSION:After treatment;improvement in lip posture with correction of incisor inclination and soft tissue profile,a class I relationship,functional occlusion and ideal overbite and overjet relationship were obtained and also observed that the wisdom teeth had erupted and reached the normal occlusal level.
