Serbest flep cerrahisi sonrası postoperatif erken dönemde
oluşabilecek venöz yetmezliğin profilaksisinde sistemik
düşük molekül ağırlıklı heparin (DMAH) kullanılmaktadır.
Belirtilen tedavi sırasında istenen etki mikrovasküler
anastomoz yapılmış flep venlerindeki oluşabilecek trombüs
ve venöz yetmezliğin önüne geçilmesidir. Bu çalışmada
Sprague Dawley sıçanlarda nörovasküler abdominal
ada flebinde oluşturulan venöz yetmezlik modelinde,
flep cildine subkutan uygulanan DMAH ile sistemik
uygulanan DMAH’nin venöz yetmezlik üzerine olan etkileri
karşılaştırıldı.
Materyal Metod: On beş sıçanda (Erkek cinsiyet, 16-
20 haftalık, 375-425 gram) 5x3cm boyutlarında iki adet
inferior epigastrik arter ve ven pediküllü nörovasküler
abdominal ada flepleri kaldırıldı epigastrik venler diseke
edilip sütür ile bağlanarak venöz staz oluşturuldu ve flepler
yerlerine sütüre edildi. Yedi saat boyunca flepler venöz
yetmezlikte bırakıldı ve yedinci saatin sonunda sıçanlar
tekrardan anestezi verilerek uyutulduktan sonra epigastrik
vendeki sütürler alındı ve flepler yerlerine sütüre edildi.
Birinci gruptaki sıçanlara antikoagülan tedavi uygulanmadı,
beş hayvandan oluşturulan on flep bu gruba dahil edildi.
İkinci gruptaki beş sıçanın sağ taraflarından kaldırılan beş flebe
subkutan antikoagülan (Enoxoparin 5mg/kg 1x1) uygulandı.
Üçüncü gruba beş sıçan ve on flep dahil edildi,
sıçanların subskapüler bölgelerinden sistemik subkutan
antikoagülan (Enoxoparin 5mg/kg 1x1) uygulandı.
-Dördüncü grup ikinci gruptaki beş sıçanın antikoagülan
tedavi uygulanmayan sol taraftaki beş flebinden oluşturuldu.
Antikoagülan tedavi ikinci, üçüncü ve dördüncü gruba
postoperatif birinci günden başlanarak beş gün boyunca
devam edildi. Beşinci gün tüm sıçanlarda oluşturulan
flepler makroskopik olarak incelenerek nekrotik olan
kısımların yüzdesi hesaplanarak farklı şekillerde uygulanan
antikoagülan tedavilerin etkinliği karşılaştırıldı. Ortalamalar
ve standart sapmalar tek yönlü ANOVA testi ile karşılaştırıldı
ve p<0,05 değeri anlamlı olarak kabul edildi.
Bulgular: Nekroz oranları 1. grupta 34±30; 2. grupta
40±41, 3. grupta 19±29, 4. grupta 36±41 bulundu ve
gruplar arasında istatistiksel olarak anlamlı fark olmadığı
saptandı.
Sonuç: Çalışmanın bulguları ışığında flep içi düşük molekül
ağırlıklı heparin uygulaması önerilmemektedir.
Systemic low molecular weight heparin (LMWH) is used in the prophylaxis of venous insufficiency that may occur in the early postoperative period after free flap surgery. The desired effect during the specified treatment is to prevent thrombus and venous insufficiency that may occur in flap veins with microvascular anastomosis. In this study, the effects of LMWH applied subcutaneously to the flap skin and LMWH applied systemically on venous insufficiency in the model of venous insufficiency created in neurovascular abdominal island flap in Sprague Dawley rats were compared. Material Method: In 15 rats (Male gender, 16-20 weeks old, 375-425 grams), two 5x3cm neurovascular abdominal island flaps with pedicle of inferior epigastric artery and vein were removed, epigastric veins were dissected and ligated with sutures to create venous stasis and the flaps were sutured to their places. . The flaps were left in venous insufficiency for seven hours, and after the rats were anesthetized again at the end of the seventh hour, the sutures in the epigastric vein were removed and the flaps were sutured in place. The rats in the first group did not receive anticoagulant treatment, ten flaps formed from five animals were included in this group. Subcutaneous anticoagulant (Enoxoparin 5mg/kg 1x1) was administered to five flaps raised from the right side of five rats in the second group. Five rats and ten flaps were included in the third group, systemic subcutaneous anticoagulant (Enoxoparin 5mg/kg 1x1) was administered from the subscapular regions of the rats. -The fourth group was formed from five flaps on the left side of the five rats in the second group that did not receive anticoagulant treatment. Anticoagulant treatment was continued for five days, starting from the first postoperative day in the second, third and fourth groups. On the fifth day, the flaps formed in all rats were examined macroscopically, and the efficacy of anticoagulant treatments applied in different ways was compared by calculating the percentage of necrotic parts. Means and standard deviations were compared with one-way ANOVA test, and p<0.05 was considered significant. Results: Necrosis rates were 34±30 in group 1; It was found 40±41 in group 2, 19±29 in group 3, 36±41 in group 4, and there was no statistically significant difference between the groups. Conclusion: In the light of the findings of the study, intra-flap low molecular weight heparin administration is not recommended.
