We report a case of severe respiratory failure in a young woman. Circulatory and respiratory instability failed to respond to inotropic medication, optimal PEEP and prone position. The use of echocardiography rapidly diagnosed a left atrial myxoma. Following the excision of the tumor, ventilatory and circulatory requirements improved dramatically.
