Ganglia of the feet are common and, considered to be the most common clinically encountered, benign soft tissue mass of the foot and ankle. Surgery is the preferred method of treatment, especially in persistant symptomatic ganglia; however, due to proximal extension of these lesions into joint spaces or tendon sheaths, recurrences are high. We present two cases with ganglia of the hallux, which underwent intralesional radiocontrast injection before excision. The distribution of the contrast agent showed the proximal extension of the lesion into the flexor tendon sheath, and enabled total excision. We recommend contrast material injection into the ganglia of the toes before surgery to establish the extent of the lesion before surgical excision, which otherwise may be underestimated, leading to incomplete removal, and hence recurrence. © 2004 European Foot and Ankle Society.
