Osteoporotik Revers Oblik İntertrokanterik Femur Kırığında 3 Farklı İnternal Fiksasyon Tekniğinin Biyomekanik Karşılaştırması

Özet

Amaç: Özellikle lateral kortekste parçalanmanın eş- lik ettiği revers oblik osteoporotik kalça kırıklarının tedavisi halen problemli ve komplikasyona açık bir cerrahi tedavidir. Çalışmanın amacı; lateral korteksi parçalanmış, reverse oblik kırık modeli üzerinde uy- gulanan 3 farklı internal fiksasyon metodunun biyo- mekanik olarak karşılaştırılmasıdır.

Yöntem: Çalışmamızda; osteoporotik kemik modeli (Sawbones, SKU: 1130-130, Malmoe-Sweden) üze- rinde lateral kortekste parçalanmanın oluşturulduğu revers oblik kırık modeli oluşturuldu. 24 adet kemik modeli üzerinde mevcut kırık komfigürasyonu yara- tılarak; 8 yapay kemik proksimal femur çivisi (PFN) (InterTAN® -Smith & Nephew, Memphis, TN) ile, 8 yapay kemik AO plak (AOP) (Ortopro, İzmir, Turkey) ile ve 8 yapay kemik ise proksimal femur anatomik kilitli plağı (PFLP) (Peri-Loc 4.5 mm, Smith & Nep- hew, Memphis, TN) ile tespit edildi. Tespit sonrası tüm femur modelleri aksiyal ve rotasyonel sertlik ile başarısızlık yükünün değerlendirilmesi için, aksi- yel yüklenme ve bending kuvvetleri ile biyomekanik laboratuarında Instron 5800R test cihazı (Instron, Canton, MA) ile teste tabi tutuldu. Kırık hattında 10 mm üzerinde deplasman ve 10 dereceden fazla angü- lasyon başarısızlık olarak kabul edildi. Biyomekanik test sonrası alınan sonuçlar tüm gruplar arasında kar- şılaştırıldı.

Bulgular: Biyomekanik analizler sonunda elde edilen başlangıç sertliği değerleri; PFN uygulanan grupta 71.7±13.5 Nm/m, AOP uygulanan grupta 46.5±13.5 Nm/m ve PFLP uygulanan grupta ise 82±34 Nm/m olarak bulunmuştur. PFLP grubunda başlangıç sertliği daha yüksek olmakla birlikte, PFN grubuna kıyasla bu yükseklik istatiksel olarak anlamlı değilken (p=0.378), AOP grubuna kıyasla fark istatistiksel olarak an- lamlıydı (p=0.005). Ardından değerlendirilen aksial sertlik değerleri; PFN grubunda 74.6±14.4 Nm/m, PFLP grubunda 77.4±10.5 Nm/m ve AOP grubunda 48.5±13.8 Nm/m olarak tespit edildi. Gruplar son olarak son yükleme testine tabi tutuldular. PFN gru- bunda ortalama yetersizlik yüklenmesi 230.2±35.8 Nm/m, PFLP grubunda 243.2±33 Nm/m ve AOP gru- bunda 187.3±28.7 Nm/m. Bu sonuçlara göre AOP ile osteosentez bu kırık modelinde biyomekanik olarak istatistiksel olarak anlamlı biçimde en başarısız bu- lunmuştur. Bununla birlikte dayanılıklılığın en yüksek olduğu grup PFLP grubu iken, PFN grubu ile yapılan karşılaştırmada istatistiksel anlamlılık elde edileme- miştir (p=0.435).

Çıkarımlar: Özellikle lateral kortekste parçalanma- nın eşlik ettiği osteoporotic kalça kırıklarında, stabil osteosentez elde edilmesi açısından PFLP ve PFN et- kin yöntemlerdir. Biyomekanik çalışmamızda PFLP ile osteosentezin biyomekanik dayanıklılığının daha fazla bulduk. Buna rağmen bu kırık tipinde hangi tek- niğin daha uygun olacağının, yeni klinik çalışmalar ile değerlendirilmesi gereklidir. 

Introduction:

The treatment of the reverse oblique osteoporotic femur fractures is still problematic and can be complicated especially that are accompanied by a fragmented lateral cortex. The aim of this study was to compare three different internal fixation methods in the osteosynthesis of osteoporotic reverse oblique intertrochanteric femur fracture models with a fragmented lateral cortex.

Materials and Methods:

A total of 24 osteoporotic femur models were obtained and divided into three groups (Group A: Proximal femoral nail, Group B: 95° angled blade plate (ABP), Group C: proximal femoral anatomic locking plate (PRLP)) with each group include eight bones. A standard fracture configuration had created as a revers oblique intertrochanteric fracture and fixed with these implants. After fixation, all femur constructs were tested with an Instron 5800R tester (Instron, Canton, MA) in the biomechanics laboratory with axial loading and bending forces to assess axial and rotational stiffness and failure load. Displacement over 10 mm and angulation greater than 10 degrees in the fracture line were considered as failure.

Results:

In the biomechanical analysis, initial stiffness values were 71.7 ± 13.5 Nm/m for PFN group, 46.5 ± 13.5 Nm/m for AOP group and 82 ± 34 Nm/m for PFLP group. The mean axial stiffness was 74.6 ± 14.4 Nm/m for PFN group, 48.5 ± 13.8 Nm/m for AOP group and 77.4 ± 10.5 Nm/m for PFLP group. Stiffness at 15 degrees of flexion was 67.7± 14.4 Nm/m for PFN group, 39.2 ± 14.2 Nm/m for AOP group and 79.3 ± 24.9 Nm/m for PFLP group. The mean failure load was 230.2 ± 35.8 Nm/m for PFN group, 187.3 ± 28.7 Nm/m for AOP group and 243.2 ± 33 Nm/m for PFLP group. In all tests AOP had statistically poorer results in comparison to the PFN and PFLP group. PFLP fixation had better biomechanical fixation results in comparison to the PFN group although the results were not statistically significant.

 

Discussion:

 

AOP fixation had insufficient biomechanical properties in comparison to the PFN and PFLP fixation. Orthopaedic surgeons should keep in mind that lateral cortex comminution brings further instability to these revers oblique intertrochanteric osteoporotic fractures and high rates of failure may be encountered due to this instability. PFLP fixation may be an alternative fixation method biomechanically for these instable fractures. Nevertheless, more clinical trials should be done in order to evaluate the effectiveness of these implants.
