Medication Adherence in Schizophrenia: Associations With Insight, Quality of Life, and Metacognition

Objectives The aim of this study was to examine medication adherence among outpatients with schizophrenia in relation to quality of life, insight, and metacognitive functioning, as well as associated sociodemographic and clinical factors.Methods This study included 84 outpatients diagnosed with schizophrenia, followed at a tertiary clinic. Sociodemographic and clinical characteristics were assessed, and standardized scales were applied to evaluate symptom severity, medication adherence, quality of life, insight, and metacognition. Group comparisons, correlation analyses, and multiple linear regression analyses were performed.Results Overall, 28.6% of patients were classified as medication nonadherent. Nonadherent patients exhibited significantly higher PANSS negative, general psychopathology, and total scores compared with adherent patients. Patients with medication adherence demonstrated significantly higher insight scores and higher quality of life. In multivariate analysis, insight emerged as the strongest independent predictor of medication adherence, exceeding the contribution of symptom severity. Metacognitive beliefs were not significantly associated with medication adherence.Conclusion Medication nonadherence remains a clinical problem in schizophrenia and is associated with greater symptom severity, reduced insight, and poorer quality of life. Insight appears to play a central role in medication adherence, independently of symptom burden. Insight-focused interventions may contribute to better medication adherence in schizophrenia.
