Orthodontic Space Opening of Early Extracted Maxillary Central Incisor
INTRODUCTION: The treatment of missing maxillary central incisors is always a challenging task, often requiring a multidisciplinary approach. This case report describes the orthodontics treatment of a patient with missing maxillary central incisor.
CASE DESCRIPTION: A 18-year-old male patient with the chief complaint of esthetic. He had history of meningitis. He gets surgical procedure at 10-year-old to extract his impacted central incisor, 26 and 36 were also extracted earlier due to the caries, the extracted teeth space was closed by mesial tipping of adjacent teeth. The maxillary dental midline was deviated to the right by 5,5 mm relative to the facial midline. The canine and molar relationship altered due to multiple extraction, maximum arch length discrepancy and bilateral posterior cross-bite. On cephalometric examination revealed skeletal Class-I relationship with normal inclination of incisors and normal growth pattern with retrochelie superior and inferior.
DISCUSSION: The Treatment options for missing central incisor are reimplantation, close the space by positioning the adjacent teeth or reopen the space allowing for prosthetic replacement and these options depend on different factors. The replacement of adjacent lateral incisor to take form of central incisor doesn’t correct the midline and posterior cross-bite. The reopening space for teeth it’s the reliable for aesthetic and ideal occlusion, despite bone graft needed for dental implant due to long-term loss of the maxillary central incisor that resulted in alveolar ridge resorption.
CONCLUSION/CLINICAL SIGNIFICANCE: Opening space with fixed appliances for implant replacement provides optimal aesthetics and function for patients where the permanent maxillary central incisor is missing.
