Abstract
Treatment of Isolated Gingival Recession in the Mandibular Region Using the Laterally Advanced Tunnel Technique: A Case Report
Aim
Gingival recession is a common periodontal condition characterized by the apical displacement of the gingival margin beyond the cementoenamel junction, resulting in exposure of the root surface. This condition may lead to esthetic concerns, dentin hypersensitivity, and difficulty in plaque control. Surgical treatment of isolated gingival recessions, particularly in the mandibular anterior region, can be challenging due to anatomical limitations. Therefore, various mucogingival surgical techniques have been developed to achieve root coverage and increase soft tissue thickness. The laterally advanced tunnel technique, when combined with a connective tissue graft, has been suggested as an effective and predictable approach for the treatment of gingival recession in the mandibular region.
Materials and Methods
A 26-year-old male patient with no systemic disease presented to our clinic with complaints of gingival recession and dentin hypersensitivity. The patient had a history of orthodontic treatment. Clinical examination revealed a 7 mm gingival recession on tooth 4.1, classified as Miller Class III and Cairo RT2, with a thin periodontal phenotype. In addition, no keratinized tissue was present in the affected area. Following non-surgical periodontal therapy and reinforcement of appropriate oral hygiene instructions, root coverage was planned using the laterally advanced tunnel technique with a de-epithelialized connective tissue graft. The graft and the flap were stabilized with 6/0 polypropylene sutures. No complications were observed during the postoperative period, and tissue healing was monitored through regular follow-up visits.
Results
At the 6-month follow-up examination, measurements showed a 6 mm increase in the width of keratinized tissue compared with baseline, and the gingival recession depth was reduced to 1 mm. The patient continues to attend routine follow-up visits at our clinic.
Conclusion
The laterally advanced tunnel technique combined with a connective tissue graft can provide effective and predictable results in the treatment of isolated gingival recession defects with a thin periodontal phenotype in the mandibular region.

Mandibular Bölgedeki İzole Diş Eti Çekilmesinin Laterale Kaydırılan Tünel Tekniği ile Tedavisi: Bir Olgu Sunumu
Amaç;
Gereç ve Yöntem;
Sistemik hastalığı bulunmayan 26 yaşında erkek hasta kliniğimize diş eti çekilmesi ve dentin hassasiyeti şikayetiyle başvurdu. Ortodontik tedavi geçmişi bulunan hastada 4.1 nolu dişinde Miller III-Cairo II sınıflamasına sahip 7mm diş eti çekilmesi ve ince periodontal fenotipe sahip olduğu tespit edildi. Ayrıca ilgili bölgede keratinize doku mevcut değildi. Cerrahi olmayan periodontal tedavi ve hastanın uygun oral hijyen talimatlarını yerine getirmesini takiben, kök yüzeyinin de-epitelize diş eti grefti kullanılarak yapılan laterale kaydırılan tünel tekniği ile kapatılması hedeflenmiştir. Operasyon bölgesi 6/0 propilen dikişlerle sabitlenmiştir. Operasyon sonrası dönemde herhangi bir komplikasyon yaşanmamış, düzenli kontroller ile doku iyileşmesi takip edilmiştir.
Bulgular;
Sonuç;
Laterale kaydırılan tünel tekniği, bağ dokusu grefti ile birlikte uygulandığında mandibular bölgede görülen, ince fenotipe sahip, izole diş eti çekilmelerinin tedavisinde etkili ve öngörülebilir sonuçlar sağlayabilen bir cerrahi yaklaşımdır.
