GLANS DUPLİKASYONU: NADİR BİR OLGU SUNUMU

Özet
Olgu Sunumu: Olgumuz doğum sonrası fark edilen penis şekil bozukluğu ve idrarı normal yerin altından yapma
şikayetleri ile tarafımıza başvuran 4,5 yaşında erkek hasta. Özgeçmişinde bilinen ailevi akdeniz ateşi ve juvenil
idiyopatik artrit tanıları ile çocuk romatoloji tarafından da takip edilmekteydi. Hastanın fizik muayenesinde normal
görünümde tek bir penis yapısının olduğu, distalinde dorsal ve ventral konumdaki glanslar üzerinde iki ayrı üretral
meatus görüntüsünün olduğu görüldü. Dorsaldeki glans boyutları daha büyük olmasına rağmen, idrar çıkışının
ventraldeki daha küçük boyutlu glanstaki mevcut meatustan sağlandığı görüldü. İşeme tek yerden ve normal
kalibrasyonda idi. İki üretral meatus yapısından kateterizasyon denendi. İşemenin gerçekleştiği ventral yüzdeki
meatus, 6 Fr beslenme sondası ile rahat bir şekilde kateterize edilebildi. Ancak dorsal yüzdeki mea görüntüsünün
kateter ilerletilemeyen kör bir fossa olduğu görüldü. Olgunun ayırıcı tanısında üretra duplikasyonu, üretra
prolapsusu, glans duplikasyonu bulunmaktaydı. Tanıyı ve tedavi planını netleştirmek için genel anestezi altında
muayene edilip tekrar kateterize edildi. Tek bir üretra varlığı teyit edilerek operasyona geçildi. Operasyon sırasında
ventraldeki daha küçük boyutlu -aksesuar- glans eksize edildi ve üretra mobilize edildi. Dorsal büyük glansın
kanatları ayrılarak üretra içerisine yerleştirildi. Glans kanatları birleştirildi. 6 Fr beslenme sondası stent olarak
bırakılarak sirkümsizyon yapıldı.
Çıkarılan materyalin -aksesuar glans- histopatolojik değerlendirmesi 'lümen yapısı içermeyen skuamöz epitel ve
subepitelyal doku' şeklinde raporlandı.
Sonuç: İzole glans duplikasyonu olarak değerlendirilen olgunun cerrahi tedavisi sorunsuz seyretmiştir. 4. ay
takibinde kozmetik ve fonksiyonel sonuçları iyi olarak değerlendirilmiştir.

Introduction: Diphallia is a very rare congenital anomaly of the phallus. Considering the embryological,
anatomical, clinical and therapeutic implications, diphallia are classified into four groups (true diphallia,
hemiphallus, pseudodiphallia, and partial duplication). Within the spectrum of diphallia, isolated glans duplication
is the least common form. Here, we will share a case that we consider as isolated glans duplication.
Case Report: Our case is a 4.5-year-old boy who applied to us with the complaints of penile deformity noticed
after birth. He was followed up by pediatric rheumatology with the known diagnoses of familial mediterranean
fever and juvenile idiopathic arthritis. In physical examination, there was a single penile structure with a normal
appearance, and two separate urethral meatus images were observed on the dorsally and ventrally located two
glans. Although the dorsal glans size were larger, it was observed that urine output was provided from the existing
meatus in the smaller sized glans ventrally. Urination was in one mea and in normal calibration. Catheterization
was attempted from two urethral meatal structures. The meatus on the ventral side where urination took place
could be easily catheterized with a 6 Fr feeding tube. However, the image of the meatus on the dorsal glans was

seen as a blind fossa in which the catheter could not be advanced. The differential diagnosis of the case included
duplication of the urethra, prolapse of the urethra, and duplication of the glans. He was examined and
recatheterized under general anesthesia to clarify the diagnosis and treatment plan. The presence of a single urethra
was confirmed and the operation was started. During the operation, the smaller sized ventral -accessory- glans was
excised and the urethra was mobilized. The wings of the dorsal greater glans were separated and the urethra was
placed inside. Glans wings approximated. Circumcision was performed by leaving the 6 Fr feeding catheter as a
stent.
The excised material’s -accessory glans- histopathological examination resulted as 'unspecified squamous
epithelium and subepithelial tissue with no lumen structure'.
Conclusion: The surgical treatment of the case, which was evaluated as an isolated glans duplication, was
uneventful. In the 4th month follow-up, the cosmetic and functional results were evaluated as good.
