Celiac disease is an autoimmune enteropathy triggered by gluten intake in genetically susceptible individuals, leading to villous atrophy and malabsorption in the small intestine. The disease presents with a highly broad and atypical clinical spectrum. Non-celiac gluten sensitivity is characterized by the absence of specific serological and histological features, despite presenting with similar symptoms. The objective of this study is to retrospectively evaluate immunological parameters requested with the suspicion of celiac disease and to compare seropositivity distribution according to pediatric and adult age groups. Laboratory data of patients presenting to Istanbul Faculty of Medicine between 2020 and 2025 were analyzed. In the 0-18 age group, anti-transglutaminase IgA positivity was 1,297 (5.07%), while in the 18-50 age group, it was 654 (2.56%). The frequency of seropositivity in the pediatric cohort rapidly approaching adult rates is a striking finding. The data strongly support that serological screening is indispensable for early diagnosis and preventing long-term autoimmune complications.
